Good Wound Care accepts referrals from:
- Individuals (self -referral)
- Family member or Carer
- Health professionals
- Funding providers
- Case managers
A referral can be made by phone, email, post or via our online form.

Referral Form
It is appreciated if referrals contain as much information as possible to facilitate timely care and collaboration and is preferred if the GP or others in the team have been advised. Referrals will be accepted from practice software or in any written format.
For your convenience, you may complete our online referral form or you can download and print the referral form for emailing. The referral form is not mandatory but can be used if preferred.
Private clients who are self-referring are also welcome to use the referral forms if desired. Wound images are always appreciated and can be uploaded or attached to your referral form.
What happens next? Good Wound Care will inform you if additional information is required in order for the referral to be triaged or progressed.
Telehealth Wound Consultations
When it comes to complex wound issues and oedemas, face to face visits are generally preferred for a better experience and quicker results. For less complex cases and routine reviews, telehealth consultations are often clinically suitable and we are happy to offer this service in the Geelong and Ballarat regions via our secure online platform.
Good Wound Care is pleased to provide a broader Telehealth service to clients who live in Victoria outside of our Geelong/Ballarat service boundary. This means more people have access to a Wound Consultant Nurse and Lymphoedema Practitioner where limited services exist or travelling far distances is not practical.
Good Wound Care’s telehealth functionality is designed to provide a simple to use web-based platform that is compliant with the Australian standards for online security and encryption. Telehealth consultations don’t require any special software or login. A link is sent to your email or phone to simply click to enter the appointment. Telehealth can also be attended via a phone call, with the assistance of photos of your wound.

Terms and Additional Service Information
- Travel to locations outside of service boundary are available by arrangement, however will incur a travel fee. Refer to service boundary map on right
- Program funded individuals must have confirmation of third party responsibility before service commencement.
- Privately funded individuals are responsible for payment of fees at the time of service
- Good Wound Care accepts payments via bank transfer, credit/debit card or cash
NB: Travel for some funded programs such as TAC and NDIS will incur travel fees in accordance with their respective pricing schedule with prior approval
- Please bear in mind that our mobile service is influenced heavily by road conditions and sometimes unpredictable situations with other clients. Good Wound Care will phone to update you on the arrival time if this is anticipated to be 20 minutes either side of the agreed visit time
- An after-hours and weekend service is available upon request for an additional fee
- Cancellations made 24 hours or less before a scheduled consult will incur a fee. This includes inability to access the property, being turned away, or if the individual is unavailable beyond a reasonable explanation and alternate billable work is not found for the estimated time spent (or in accordance with current NDIS pricing arrangements).
